[opencms-dev] implementaton of custom form

Hi List,

We are trying to migrate an html form from our old site to our new opencms site, as example of the old form can be found here https://forms.manitoba-ehealth.ca/getcheckedmb/resources.html , I cannot seem to understand, how to rebuild this form within opencms, I had thought to rebuild it using html in a flexible content type, but cannot get the form handling to work that way. Next tried using the webform module, and though I can get the demographics information part of the form to work with the webform module, the select quanitity sections of the form do not.
