My  understanding of your situation is that your parents set up an investment portfolio or account for  you when you were born. That since  its creation  it is  has lost half of its value. 

In addition, you have a savings account in your name in the sum  of  4k. 

In regards to  your Fasfa,  it will ask you  your assets and that of your parents.  


Depending on whose name  those  accounts are  under, you will  put the sums either  in  your  parents or yours. 

The investment sounds like sir thing that will  be listed in your parents. The savings would be under yours. 

Had  to do some research when I filled mine out.
