[opencms-dev] Creating a template JSP - Step 2

re "screen.css": (not 100% sure, but) I assume you're right; just try it, you can always change it later on

re "template.provider": are you in the "Individual properties" tab of the dialog?
IIRC I never had to add this property (but had to change its value)
